You must currently be a student at Appalachian State University to complete this survey. Your responses will remain confidential, and your participation is totally voluntary. You may quit this survey at any time, even after you have started the survey. 

If you would like to enter a drawing for one of five $50 Amazon gift cards, there will be a separate Google Form at the end of the survey for you to enter your name and email address. You must complete this form to receive a gift card. Any information entered on the Google Form will not be linked in any way to your survey responses.
